About 3y Health and the Engineering Philosophy
3y Health is a modern healthcare technology company focused on reimagining healthcare operations and patient management. In traditional healthcare systems, clinicians and administrative operators waste countless hours dealing with insurance verification, prior authorizations, patient intake documentation, and repetitive claims work. 3y Health changes this reality by introducing intelligent automations, intuitive clinician tooling, and robust AI agents to solve operational bottlenecks.
What makes the 3y Health engineering culture truly distinct is its flat structure. There are no deeply siloed sub-teams or bureaucratic layers slowing down development. Every engineer works directly across the whole technical platform. Even more fascinating is their progressive adoption of developer tooling: the engineering team relies on AI coding agents such as Claude Code, Cursor, and Codex on an everyday basis. If you enjoy shipping features fast with high autonomy and modern AI pair-programmers, you will feel right at home here.
What Your Day-to-Day Responsibilities Will Look Like
As a Member of Technical Staff, you are expected to take complete ownership of features from start to finish. You will not simply be given Jira tickets with rigid instructions; you will be talking to real users, identifying problems, architecting the solution, and observing its behavior in production. Your responsibilities include:
- Designing AI Agents and Automated Pipelines: Build autonomous workflows tailored for everyday clinical operations, including patient onboarding, real-time insurance verification, automated medical billing, and clinical documentation assistance.
- Building Multi-Platform Features: Create intuitive, reliable interfaces for clinicians, operational staff, patients, and their family members using modern web frameworks like React, Next.js, and mobile solutions using React Native.
- Taking Ambiguous Projects From Zero to One: Sit down with clinicians and non-technical operators to understand core pain points, turn vague ideas into detailed technical specs, write clean code, and roll out the initial versions.
- Microservices and Data Infrastructure: Architect back-end services, manage relational schemas in PostgreSQL and Snowflake, design gRPC/REST APIs, and write declarative Infrastructure as Code using Terraform.
- Production Ownership and Monitoring: Own your services after deployment by tracking uptime, setting up Datadog alerts, evaluating model accuracy, and handling incident response proactively.
- Ensuring Healthcare Compliance & Data Privacy: Handle Protected Health Information (PHI) carefully by adhering to strict HIPAA guidelines, zero-PHI logging practices, and auditable data controls.
- Supercharging Team Productivity: Build custom plugins, MCP (Model Context Protocol) servers, and AI review bots that assist team members while coding with Claude Code or Codex.
Tech Stack in Detail
Here is a quick look at the tech ecosystem you will touch on the job:
- Backend: Python (primary language for new services), Kotlin with Hasura (GraphQL), TypeScript (NestJS), and occasional Go services.
- Frontend & Apps: React, Next.js, TypeScript, and React Native for mobile.
- Storage & Data: PostgreSQL (utilizing pgvector for embeddings and vector search) and Snowflake for warehousing.
- Orchestration: Temporal for long-running durable workflows, with gRPC and Protobuf for clear contracts.
- Cloud & DevOps: AWS (ECS Fargate, RDS), Terraform, GitHub Actions, Datadog, LaunchDarkly.
- AI Ecosystem: Anthropic Claude and OpenAI models, Claude Code, Cursor, Codex, and custom in-house MCP servers.
Eligibility Criteria and Qualifications
To be considered for this opening, candidates should meet the following baseline expectations:
- Experience: At least 3+ years of professional hands-on software development experience shipping web services or applications to production.
- Education: A degree in Computer Science, Information Technology, Software Engineering, or equivalent practical hands-on experience in the industry.
- Full-Stack Mastery: High comfort moving across the stack in Python and TypeScript, connecting databases, handling back-end logic, and tweaking frontend components.
- Hands-on LLM Experience: Familiarity with tool-calling, agent orchestration (e.g., LangGraph or custom agent SDKs), prompt evaluations, and structured JSON outputs from language models.
- AI-Native Developer Habits: Active daily experience with AI coding tools like Claude Code, Codex, or Cursor. You must know how to set up project rules, skills, subagents, and critically verify the output.
- Communication & Autonomy: Strong written communication skills since the team is globally distributed and relies heavily on written pull requests, specifications, and issues.
Bonus Points (Nice-to-Haves)
You will gain an extra advantage if you bring experience in any of these areas:
- Previous exposure to healthcare tech, HIPAA compliance, electronic health record (EHR) integrations, or HL7/FHIR standards.
- Experience running Temporal or another durable event-driven workflow engine.
- Building mobile applications using React Native.
- Creating custom Model Context Protocol (MCP) servers, plugins, or agent harnesses.
- Active open-source contributions or impactful side projects.
Hiring Process Overview
While the hiring workflow may be customized as you move forward, typical recruitment steps for this position include:
- Resume & Profile Review: The engineering leads review your prior experience, past projects, GitHub profiles, and answers to initial screening questions.
- Introductory Screening: A friendly 30-minute conversation to discuss your background, team fit, and experience with modern AI development tooling.
- Technical & Architecture Discussion: A deep dive into full-stack architecture, system design, data modeling, and practical problem-solving using AI tools.
- Hands-on Pairing Session: A collaborative session where you build or debug something realistic, showing your natural workflow and how you use coding assistants.
- Offer & Onboarding: Final alignment on contract terms, compensation, and setting up your remote developer environment.
Tips for Candidates to Stand Out
- Highlight Your Claude Code / Cursor Workflow: The job description explicitly emphasizes AI coding agents. Mention in your application how you use these tools to boost your delivery speed and review code critically.
- Share Real Demos or Repos: If you have written MCP servers, custom agents, or full-stack side projects, include working links in your resume or cover letter.
- Demonstrate High Agency: Share instances where you tackled open-ended requirements without waiting for comprehensive documentation or step-by-step guidance.
